The Medical Social Worker is responsible for the provision of consultation and service to homebound individuals in the need of assistance with the solution of complex social and emotional problems, which impede management of medical regimen. Medical Social Worker (MSW) - Home Health $5,000 sign on bonus Position Summary The Medical Social Worker (MSW) provides counseling, care coordination, and community resource support to homebound patients and their caregivers. Working collaboratively with physicians and interdisciplinary care teams, the MSW helps patients overcome social, emotional, and environmental barriers that may impact their health, safety, and quality of life. Key Responsibilities Conduct comprehensive psychosocial assessments and develop individualized care plans. Provide counseling and emotional support to patients and caregivers facing illness, disability, or life transitions. Connect patients and families with community resources, financial assistance programs, and support services. Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care. Educate patients and caregivers on coping strategies, available resources, and care options. Monitor patient progress and adjust care plans as needed. Maintain accurate and timely clinical documentation. Participate in interdisciplinary team meetings, care conferences, and quality improvement initiatives. Support patients with complex social and emotional needs to improve health outcomes and independence. Qualifications Master's Degree in Social Work (MSW) from an accredited program required. Current Social Work licensure as required by state regulations. Minimum two years of healthcare social work experience preferred. CPR certification required. Strong assessment, counseling, communication, and care coordination skills. Valid driver's license and reliable transportation. Why Join Us?
