About the Bachelor level Therapist Role Central is seeking a compassionate and engaging Bachelor's-Level Therapist to join our Older Adult Partial Hospitalization Program (PHP). Responsibilities Therapeutic Services:

Facilitate daily psychoeducational and psychotherapy groups focused on coping skills, depression, anxiety, grief, and life adjustments.

Individual Counseling:

Provide tailored, person-centered individual therapy sessions to address personal goals and treatment needs.

Treatment Planning:

Develop, implement, and review patient-centered treatment plans with measurable objectives.

Interdisciplinary Collaboration:

Work closely with psychiatrists, nurses, case managers, and other professionals to ensure cohesive care, attending regular treatment team meetings.

Documentation:

Maintain timely and accurate clinical documentation, including progress notes, treatment plans, and discharge summaries in compliance with regulations (e.g., Joint Commission, CMS).

Discharge Planning:

Assist in managing the transition from acute care to lower levels of care Qualifications Education Bachelor's degree Psychology or related field Experience & Skills Geriatric and Group experience a plus. Strong interpersonal skills with a nonjudgmental, client-centered approach Comfortable with digital tools and basic technology use (tablets, phones, computers) Ability to work both independently and as part of a multidisciplinary team Monitor needs of the consumers throughout the program day.
