Job Description

We're hiring a Behavioral Health Assistant for our New Madrid & Sikeston Locations! The Behavioral Health Assistant supports the Primary Care Health Home (PCHH) program by assisting behavioral health providers and care teams in coordinating comprehensive, patient-centered care for individuals with behavioral health and chronic medical conditions. This position supports care coordination activities, patient engagement, data tracking, outreach, and documentation to improve patient outcomes and support PCHH program requirements.
Qualifications:
Bachelor's degree in psychology or social work preferred. 6 months of Behavioral Health experience required•
