Summary:
Under the direction and reporting to the assigned Registered Nurse the LPN provides nursing care to patients in their homes, educating patients and families on their care plan, and collaborating with the healthcare team to ensure optimal patient outcomes, all while adhering to organizational policies and procedures including the patient's home environment.
Education and Work Experience:
Graduate from an accredited practical nursing program. Current LPN License in the State of Wisconsin. Minimum of 1 year experience in clinical setting, preferably in home healthcare.
Nature & Scope-Principal Areas of Responsibilities:
(This list of duties and responsibilities is not all inclusive and may be expanded to include other duties and responsibilities, as management may deem necessary from time to time) Reports any significant changes to the supervising nurse or physician. Assists with administering and interrupting Tuberculosis Skin Tests as trained and supervised by RN. Conducts Personal Care Worker new hire and annual orientation. Conducts Meet and Greets for Personal Care Worker at patient's house. Conducts training and education for Personal Care Workers specific to client's plan of care. Provide patient and family education regarding disease management, care techniques, and self-care practices. Maintain accurate and detailed patient records, including clinical observations, interventions, and progress notes, adhering to agency documentation guidelines. Work closely with the interdisciplinary healthcare team including physicians, registered nurses, physical therapists, and social workers to coordinate patient care plans. Answer patients' calls and determine how to assist them. Adhere to all applicable state and federal regulations, HIPAA guidelines, and agency policies regarding patient privacy and safety.
Knowledge/Special Skills/Abilities Needed:
Effective oral and written communications and presentation skills are essential, as are interpersonal skills. Exhibits advanced professional knowledge and leadership skills. The ability to make administrative and procedural decisions and judgments on sensitive, confidential issues is required. The ability to work effectively in a team environment is required. The ability to gather and analyze statistical data and generate reports and to plan, coordinate and administer complex administrative systems and processes is necessary. PC Skills to include MS Office software, such as Excel and Word, and Electronic Health Records. The ability to work with frequent, established deadlines is required. The ability to develop, plan, and implement short- and long-range goals is required. Knowledge of Federal, State and HIPPA regulations. Valid driver's license and reliable transportation to travel between patients' homes Bilingual in Spanish and English
