Job Summary Eligibility Specialist is primarily responsible for assisting persons requesting services in the Community Developmental Disabilities Organization (CDDO) catchment area for intellectually and developmentally disabled individuals. Employee will coordinate and complete documentation for service referrals and assist individuals in exploring funding resources. Eligibility Specialist will coordinate scheduling and complete all Functional Assessments for the CDDO area. Complete confidentiality is required. Directly responsible to the CDDO Director. Duties Complete initial Functional Assessment on all individuals determined eligible for I/DD Services and annual assessments as needed. Serve as the Single Point of Application, Determination, and Referral for Miami, Linn, and Anderson Counties. Process all referrals according to locally developed policies to determine whether an individual is eligible or ineligible for Intellectual/Developmental Disabilities (I/DD) Services. Complete courtesy screenings for other requesting CDDO's. Assist with the completion of required reports, committee requests, and functional assessment documentation. Impartially share information about all affiliated service providers available in the region. Coordinate tours of available providers for eligible individuals upon request. Participate in local

Meetings and the Quality Assurance Committee. Update affiliated providers monthly of status of all referrals. Coordinate and/or complete documentation for all referrals in addition to documentation/ minutes for all referral meetings. Provide professional representation of the CDDO to the other community professionals through meetings and contacts. Complete PASRR screenings. Qualifications Eligibility Specialist must have a High School Diploma/GED. A minimum of six months experience in the field of intellectual/developmental disabilities. A bachelor's degree or additional experience in the field of I/DD. Experience may substitute for the required education at the rate of six months of experience for each semester. Working knowledge of Developmental Disabilities Reform Act, Article 63 and Article 64, which govern Home and Community Base Services (HCBS) required. Prefer two to three years of related job and administrative experience. Valid Driver's License and the ability to be covered by corporate vehicle insurance. Must have good organizational skills, a professional manner and practice complete confidentiality.

Further educational/certification requirements may be assigned as approved by the CDDO Director and the Council of Community Members. This position is classified as a non-exempt position. Wages will be paid at an hourly rate. The general hours of work are 8:00 a.m. - 4:00 p.m., Monday through Friday. Hours may vary as tasks require.
