Social Worker Opportunity:

Full-time, permanent position Monday-Friday schedule Provide direct support and case management services to assigned clients The Social Worker will monitor and improve the patient experience by focusing on quality, timeliness, and continuity of care The Social Worker collaborate with multidisciplinary teams to coordinate patient care, lead case conferences, and provide clinical consultation to staff and families.

Social Worker Qualifications:

Must be a Licensed Social Worker (LICSW or LCSW) Background in hospital setting is preferred, but not required
