The COO will oversee daily business operations, implement operational excellence initiatives, and shape strategic management practices to ensure efficiency, profitability, and long-term success. This role requires an energetic leader with a passion for managing large-scale organizational change, fostering innovation, and inspiring high-performing teams. As a key member of the executive leadership team, the COO will translate strategic plans into actionable programs that propel the organization forward. About the Role We are a multi-location outpatient behavioral health and psychiatric group practice, paired with an active real estate investment operation spanning residential and commercial properties across multiple states. We're looking for a Chief Operating Officer to bring corporate-level structure and oversight to both sides of the business — someone who can run day-to-day healthcare operations with the same rigor they'd bring to underwriting a deal. This is a rare dual-scope role for someone who wants ownership-level responsibility without the capital risk, working directly with the founding partners to professionalize and scale both businesses. Key Responsibilities Practice Operations Oversee daily operations across all clinical locations, including staffing, scheduling, provider pay administration, and workflow efficiency Manage compliance and audit response (insurance audits, REMS/CAPA plans, credentialing across multiple states) Track and report on program-level profitability (including specialty service lines) Serve as primary liaison between clinical staff, billing/RCM functions, and ownership Support HR functions: hiring, onboarding, performance management, and provider retention Manage vendor, insurance, and payer relationships Real Estate Portfolio Operations Oversee day-to-day management of the real estate investment portfolio, including due diligence coordination, renovation project tracking, and lender relationships Support acquisition analysis (comps, DSCR/cap rate/cash-on-cash modeling) in partnership with ownership Manage entity-level administration across multiple LLCs (operating agreements, leases, compliance filings) Coordinate with contractors, property managers, and hard money/private lenders Track portfolio performance and report on cash flow, project timelines, and budget adherence Cross-Functional / Strategic Build reporting systems and dashboards that give ownership real-time visibility into both business lines Identify process improvements and implement systems/software to reduce administrative burden Manage relationships with outside accountants, attorneys, and consultants Represent the organization professionally in dealings with banks, auditors, and regulatory bodies Qualifications Bachelor's degree required; MBA, MHA, or equivalent advanced degree preferred 5+ years in operations management, practice administration, or a comparable multi-entity leadership role Experience in healthcare operations (medical/behavioral health practice management) strongly preferred Familiarity with real estate investment fundamentals (financing structures, renovation project management) a significant plus Strong financial literacy — comfortable reading P&Ls, building budgets, and evaluating deal economics Excellent organizational and communication skills; able to manage competing priorities across two distinct industries Proficiency with practice management/EHR systems and standard business software (Excel required) Discretion and trustworthiness — this role will have visibility into sensitive financial and clinical information What We Offer Competitive base salary plus performance bonus Direct access to ownership and real influence over how both businesses are run Opportunity to build and shape operational systems from the ground up health insurance, PTO, retirement plan, and more
