Applicants must live within a 50-mile radius of Lawton, Oklahoma. Oklahoma Veterans United is seeking a compassionate and organized Case Manager to support veterans and their families through the SSG Fox Suicide Prevention Grant Program. This role provides direct case management, resource navigation, and ongoing support to help veterans access critical services and improve their quality of life. Key Responsibilities Conduct eligibility screenings, needs assessments, and service planning. Provide ongoing case management and bi-weekly participant meetings. Connect veterans and families to resources such as housing, employment, healthcare, behavioral health services, transportation, and financial assistance. Assist veterans with accessing VA and community-based services. Maintain accurate case notes, assessments, and program documentation. Monitor participant progress and provide follow-up support. Build relationships with community partners and veteran-serving organizations. Participate in team meetings, trainings, outreach events, and community engagement activities. Attend in-person and virtual program conferences, team meetings, community meetings, trainings, and events as required to support program goals and community engagement. Qualifications Bachelor's degree in Human Services, Social Services, Criminal Justice, Education, Behavioral Health, or a related field. Minimum two years of relevant professional experience. Strong communication, organization, and time-management skills. Proficiency with Microsoft 365 and case management/data tracking systems. Valid driver's license and ability to travel within assigned service areas. Why Join OKVU?
