This position is in the Special Diabetes Program for Indians (SDPI) section, Health Department, Fort Mojave Indian Health Center, Mohave Valley, AZ. The
FMIHC SDPI
program is to provide diabetes treatment and/or prevention activities and/or services for the tribe. The Diabetes Program Assistant/Outreach will assist in the SDPI program. This is a group-based program consisting of weekly sessions led by trained personnel who facilitates a small group of people with similar goals. Discussion topics include healthy eating, increasing physical activity, reducing stress and how behavior changes can benefit health. This program is designed for people who are diagnosed with type 2 diabetes, are at high risk for type 2 diabetes, and want to lower their risk.

Experience :
Diabetes Case Management Experience and working with Tribal Communities preferred
