2996670918-2 Apply for this job now Job Description Hartford Healthcare is seeking a Wound Program Specialist to lead and coordinate evidence-based wound care services across our Healthcare & Medical Services department. In this role, you will oversee wound care protocols, conduct advanced wound assessments, collaborate with interdisciplinary teams, and support staff education to improve patient outcomes. You will analyze quality metrics, implement best practices, and optimize care pathways. Responsibilities Lead and coordinate the wound care program across designated units or facilities Perform comprehensive wound assessments and develop evidence-based treatment plans Collaborate with physicians, nurses, therapists, and other disciplines to optimize wound management Educate and mentor clinical staff on wound care protocols, products, and documentation standards Monitor and analyze wound-related quality indicators and outcomes Develop and update policies, procedures, and clinical pathways for wound care Consult on complex wound cases and product selection Ensure regulatory compliance and adherence to clinical best practices Engage patients and families in wound care education and self-management Participate in performance improvement initiatives and program reporting Required Skills Advanced wound assessment and management Knowledge of wound care products and therapies (e.g., NPWT, advanced dressings) Care planning and clinical documentation (EMR/EHR) Interdisciplinary collaboration and consultation Clinical education and staff training Quality improvement and outcomes measurement Infection prevention and control principles Patient and family education Regulatory and accreditation standards related to wound care Data analysis and report preparation
