Full Job Description Role Overview:
A Physical Therapist acting as a Digital Health Specialist, primarily working in outpatient settings with a hybrid model of in-clinic and remote care.
Key Responsibilities:
Develop and deliver personalized treatment plans for patients with orthopedic and neurologic conditions. Provide virtual care through telehealth platforms, including evaluations, assessments, and remote monitoring. Objectively measure patient outcomes using specialized software and document all clinical activities accurately. Support operations by facilitating digital health services and ensuring patient care and staffing needs are met. Coordinate with clinicians and serve as a liaison to optimize remote service delivery and program growth. Maintain regular reporting and analyze regional service metrics; participate in program development and marketing initiatives.
Qualifications & Skills:
Licensed in physical therapy, with a diploma from a CAPTE-approved program. Experience in outpatient settings; telehealth experience preferred. Strong dedication to providing high-quality patient care. Proficiency with digital health tools and remote therapeutic monitoring. Excellent communication skills to collaborate with providers and support digital program development.
