Team Leader - Direct Care

Job Description

Community Homes is looking for a motivated, compassionate professional to join our team as a Team Leader. This position offers the opportunity to mentor staff, support individuals in achieving their goals, and help create a positive, rewarding environment for both residents and staff. Position Overview The Team Leader serves as a role model for Direct Support Professionals (DSPs), promotes a positive team culture, and helps individuals achieve their personal goals while maintaining a safe, healthy, and supportive living environment. The Team Leader is responsible for overseeing the daily operations of a residential program while ensuring individuals receive high-quality, person-centered support that promotes independence, dignity, and community inclusion. This role coordinates medical appointments and healthcare services, manages household operations and finances, maintains compliance with agency and regulatory requirements, and supports the implementation of individual service plans. The Team Leader also provides leadership to Direct Support Professionals through scheduling, training, coaching, and performance support, while maintaining accurate documentation and fostering a safe, positive, and supportive home environment. Candidates should possess strong leadership, organizational, and communication skills, experience working with individuals with intellectual and/or developmental disabilities or related populations, and a commitment to delivering exceptional care and services. Pay:
$19.00 - $22.00 per hour
