OverSightMD is looking for Lead Care Managers to join our Enhanced Care Management (ECM) team in Merced Ca. We are looking for people with "Lived Experience"—people who have walked a mile in the shoes of the members we serve. If you are a natural advocate, a problem solver, and someone who genuinely loves to help others, we want to train you to be a professional Care Manager. Who We Are Looking For We are looking for "Community Champions." You might be a perfect fit if: You are a
Caregiver:
You have experience caring for aging parents, grandparents, or family members with disabilities. You know how hard it is to manage medications and appointments. You are an
Advocate:
You have navigated the healthcare system for yourself, a child, or a loved one. You know how to ask doctors the right questions. You Have "Lived Experience": You or a loved one have experienced homelessness, substance use recovery, the justice system, or mental health challenges. You understand the stigma and the barriers.
You Volunteer:
You spend time at food pantries, homeless shelters, church groups, or community events because you care about your neighbors.
Required Skills Bilingual Preferred:
Must be fluent in Spanish.
Tech Savvy:
You must have basic computer skills. You will be documenting your visits and member stories on a computer.
Mobile:
You must have a reliable vehicle, a valid driver's license, and insurance. This is a field-based role—you will be visiting members in their homes, at shelters, or in the community (mileage is paid!). What You Will Do As a Lead Care Manager, you are the bridge between the healthcare system and the real world. Your job is to build trust.
Build Relationships:
Visit members where they are—whether that's a home, a shelter, or a coffee shop. Listen to their stories without judgment.
Advocate:
Go to doctor's appointments with members to ensure they are heard and understood. Connect to
Resources:
Help members find housing support, food banks, transportation, or legal aid.
Transition Support:
Help members move safely from the hospital or a nursing facility back into their own homes.
Empower:
Teach members how to manage their own health, reminding them that they are capable and worthy of care. The People We Serve You will be working with Merced County residents who simply need a helping hand to manage their health. Most of our members are everyday people struggling with chronic conditions who need support staying on top of doctor's appointments, medications, and follow-ups.
Families, Youth & Seniors:
Helping aging parents stay independent at home or ensuring foster youth get the care they need.
Pregnant & Postpartum Moms:
Supporting healthy pregnancies and helping new moms navigate the healthcare system. Neighbors with
Chronic Health Needs:
Helping people manage things like diabetes or heart disease so they can stay healthy and out of the hospital. Community Members seeking
Stability:
