The Supported Community Living Manager oversees Lakewood Center's Supported Community Living program for adults with serious mental illness who can live independently with scheduled support. Using a PACT-informed approach, the Manager coordinates psychiatric care, housing stability, community resources, activities, and individualized independence goals while maintaining a clear distinction between supported community living and residential treatment. Candidate will possess Master's degree or professional behavioral-health licensure preferred, or Bachelor's degree in social work, psychology, counseling, human services, healthcare administration, public health, or a related field. At least three years of experience working with adults with serious mental illness in community-based, residential, case-management, psychosocial rehabilitation, or related services. At least one year of leadership, supervisory, or program-coordination experience. Experience coordinating services among providers, families, and community organizations. Knowledge of serious mental illness, recovery-oriented care, crisis response, professional boundaries, confidentiality, and documentation requirements. Experience with ACT, PACT, FACT, intensive case management, or supportive housing. Experience with program development, admissions, housing coordination, staff supervision, or quality improvement. Knowledge of Central Florida behavioral-health, housing, transportation, benefits, and crisis resources. This position requires regular travel among participant apartments, the Lakewood Center campus, and community locations. Some evening or weekend availability may be required for activities, urgent coordination, or other program needs. Certification in CPR and First Aid, as well as required
HIV/AIDS
training within 90 days of employment. Ability to stand or walk during most of the 8 hour shift and a minimum lifting capacity of 30 pounds. Must possess a valid/clean Florida driver's license. Consent to TB testing prior to employment, ACHA Level II Background check ( https://info.flclearinghouse.com/ ), and favorable references/screenings.
