In person with regular local travel About the Role Disability Network of Mid-Michigan is seeking a Community Transition Specialist to support individuals who are transitioning from nursing facilities back into community-based living. In this role, you'll work directly with individuals to understand their needs, develop personalized transition plans, connect them with community resources, and provide continued support as they work toward greater independence. What You'll Do Support individuals transitioning from nursing facilities into community living Meet with individuals to identify needs, goals, and available supports Develop and maintain individualized support and transition plans Connect individuals with community programs, services, housing resources, and other supports Coordinate with nursing facilities, service providers, and community partners Conduct outreach and maintain regular communication with individuals through phone calls and in-person visits Maintain accurate and timely case documentation and reports Manage a caseload and track transition timelines and progress Travel regularly to nursing facilities, homes, and community locations throughout the service area What We're Looking For Strong communication and relationship-building skills Excellent organization, time management, and follow-through Ability to manage multiple cases, deadlines, and priorities Understanding of person-centered services and consumer choice Familiarity with community resources and human services Ability to work independently while collaborating with a team Comfortable conducting outreach and communicating with community partners Ability to maintain confidentiality and follow HIPAA and mandated reporting requirements Comfortable using computers for documentation, communication, and tracking Qualifications Candidates must meet one of the following: Current Registered Nurse (RN) license in Michigan Current Social Worker license in Michigan Bachelor's degree in a health or human services-related field Community Health Worker certification and at least three years of experience providing health or social services Candidates should also have experience working with community resources and an understanding of the Independent Living Philosophy. Schedule & Travel This is a full-time position . Some flexibility outside of standard business hours may occasionally be needed. Regular travel is required to nursing facilities, individuals' homes, and community locations. Candidates must be comfortable visiting a variety of environments, including locations that may require using stairs.
