Community Living Program Manager

Job Description

Job Summary:
The program manager is responsible for the daily management and oversight of the community living program. This position ensures that services promote independence, inclusion, and personal growth while meeting all regulatory, agency, and quality standards. The Program Manager provides leadership to the House Managers and Direct Support Professional, coordinates individuals support, and works closely with families, guardians, and support coordinators to ensure person-centered care. Maintain a safe, supportive, and inclusive living environment for all individuals served. The Community Living Program Manager primarily works in a combination of office and residential sitting. Maintain confidentiality of all individuals' Protected Health Information used for treatment, payment, health care, etc. as set up by the Health Insurance Portability Accountability Act of 1996. (HIPAA). Maintain individual and program confidentiality
ESSENTIAL DUTIES AND RESPONSIBILITIES
Oversee daily operations of community living sites, including staffing, scheduling, documentation, and compliance of applicable regulations, CARF accreditation standards, and funding requirements. Manage program budgets, monitor expenses, and ensure responsible use of individual and agency resources. Monitor and document individual's progress, health, and behavioral support needs. Respond to emergencies, incidents, and after-hour needs as required. Perform other duties as assigned to support the mission and operations of the agency. Participate in planning, improvement of the program to enhance service quality, and the growth of the department. The Program Manager promotes independence, inclusion, and quality of life for the people served while supporting and mentoring program staff. Additional Specific Responsibilities include the following. 1. Communicate with the Community RN about concerns and findings, resolve any issues. 2. Supervise and support house managers and direct support professionals, a minimum of 15 hours per week, to maintain high quality service delivery and staff engagement. 3. Ensure 100% of staff worked have been observed each month. 4. Assist with direct care and staff coverage, providing a minimum of 10 hours per week with vacant coverage prior to overtime being used. 5. Ensure all services follow person-centered plans, individual support plans and agency policies. 6. Conduct monthly staff meetings to share updates, review procedures, and address training or program needs. 7. Hold house meetings, quarterly ( 4 per year) and when issues arise, with the individuals served and staff to discuss program goals, concerns, and quality improvements. 8. Overseeing scheduling, staff training, and daily documentation for the program. 9. Abide by ICAN Personnel Policies as a condition of employment. 10. Model positive, respectful, and professional behavior always. 11. Participate in meetings and committees as directed, not limited to the Crisis team, Record Review 12. Oversee the Monthly Summary process - Overseeing the completion of all programs monthly summaries and ensuring their accuracy. Ensuring monthly summaries are sent to the appropriate service coordinator by the 15th of the month. 13. Billing-Oversee accurate billing practices 14. Attend IP meetings to ensure all intake and annual paperwork is signed and in electronic files as well as in hard copy binders kept onsite. 15. Update and maintain all Setworks information including fixing any problems or issues with the system.
SUPERVISORY RESPONSIBILITIES
The Community Living Program Manager is responsible for the supervisory of all direct support staff and house managers to ensure high quality services. Conduct monthly meetings with Program Mangers in the absence of the Director of Services, to discuss program information.

EXPERIENCE
Completion of a bachelor's degree or have at least a High School Diploma or GED and 5 years' experience with mental or developmental disabilities. Ability to effectively supervise, train, and motivate program level and subordinate staff. Ability to assess problems and use good judgement to implement effective solutions.
CERTIFICATES, LICENSES, REGISTRATIONS
The Assistant Director of Community Services must be an insurable driver with Class E license as determined by the company's underwriter. This person must be able to successfully complete and pass; Level 1 Medication Assistant (L1MA) CPR and First Aid Course, Abuse and Neglect Training, Positive Behavior Support, Missouri Quality Outcomes, Person Centered Planning, and 14 hours of employment curriculum training, approved by the Division of

Pay:
$54,500.00 - $58,500.00 per year
