Encompass Health seeks a Case Manager to coordinate patient-centered rehabilitation services in our inpatient hospital. In this role, you'll collaborate with an interdisciplinary team to assess patient needs, develop and manage care plans, facilitate transitions of care, and ensure continuity of services across our rehabilitation, home health, and hospice network. You will advocate for patients and families, support discharge planning, manage utilization and documentation, and promote quality outcomes. Responsibilities Assess patients' clinical and psychosocial needs to develop individualized rehab care plans Coordinate interdisciplinary team meetings and ongoing care plan updates Facilitate discharge planning and smooth transitions to home health or hospice services Educate patients and families on care plans, resources, and post-discharge needs Ensure appropriate utilization of services and compliance with payer requirements Maintain accurate, timely clinical and case management documentation Collaborate with physicians, therapists, and nursing staff to optimize outcomes Advocate for patient rights, safety, and quality of care Monitor patient progress and adjust care coordination as needed Support quality improvement and performance metrics within the unit Required Skills Clinical assessment and nursing skills Care and discharge planning Case management and care coordination Knowledge of rehabilitation and post-acute care Utilization review and payer requirements Electronic health record (EHR) documentation Interdisciplinary team collaboration Patient and family education Time management and organization Regulatory and compliance knowledge (CMS, Joint Commission)
