We're looking for a detail-oriented, dependable, and compassionate Clinical Operations & Medication Coordinator to help ensure our residents receive safe, accurate, and well-coordinated care behind the scenes.
Key Responsibilities:
Medication Management Review physician orders and process medication changes Update medication records and resident Cardex Coordinate with the pharmacy regarding medication orders Remove discontinued medications from medication packs (per facility policy) Maintain accurate medication documentation Track physician orders from receipt through completion to ensure no follow-up is missed. Clinical Coordination Review physician visit documentation and process new orders Communicate with physicians when clarification is needed Coordinate weekly laboratory requests and track laboratory results Follow up on physician orders and laboratory findings Assist with the medical portion of resident admissions Documentation & Administrative Support Maintain resident clinical records and documentation Process hospital and emergency room return paperwork Support compliance documentation and regulatory requirements Perform other related duties as assigned by the Administrator.
Qualifications:
Connecticut Medication Certification required Ability to read, understand, and accurately implement physician orders Experience in assisted living, residential care, or long-term care preferred Strong organizational and documentation skills Excellent communication and attention to detail Comfortable working with electronic records and computer systems Knowledge of pharmacy coordination and state regulations is a plus Dependable, professional, and team-oriented If you're passionate about supporting exceptional resident care through organization, attention to detail, and teamwork, we'd love to meet you!
