Job Description

Job Summary We are seeking a dynamic and bilingual (English/Spanish) Home Care Coordinator / Scheduler to join our dedicated healthcare team. In this vital role, you will serve as the primary point of contact for clients, caregivers, and healthcare providers, ensuring seamless scheduling and communication. Your energetic approach will help coordinate home care services efficiently, promote positive client experiences, and support the overall revenue cycle management. This position offers an exciting opportunity to make a meaningful difference in clients' lives while working in a fast-paced, rewarding environment. Duties Coordinate and maintain HHA/PCA client schedules Match caregivers with clients based on availability, location and client needs Communicate professionally with clients, families and caregivers Monitor schedules and help prevent gaps in client care Work closely with intake, nursing, HR, authorization and billing team Assist with EVV-related scheduling issues Handle multiple priorities in a fast-paced home care environment Experience Experience scheduling HHAs, PCAs or caregivers Understanding of the day-to-day operations of a home care agency Strong problem-solving skills when dealing with call-outs and coverage Must have great customer service and communication skills Able to manage multiple cases and priorities at the same time Strong organizational and follow-up skills Bilingual English/Spanish required HHAeXchange or other home care/EVV software experience is a plus What Makes Someone Successful Here We are looking for someone who takes ownership of their cases . When a caregiver calls out, you don't simply document the problem—you work toward a solution. When a client needs assistance, you follow through. When something needs attention, you communicate with the appropriate team member. You will be joining an established Hudson County home care agency with an experienced office team and opportunities to grow as the organization continues to develop. If you have worked in home care coordination or scheduling and enjoy solving problems, supporting caregivers and making sure clients receive dependable care, we'd like to hear from you.
Pay:
$20.00 - $23.00 per hour
