Care Coordinator

Job Description

The Resident Care Coordinator works in collaboration with the Executive Director and Health & Wellness Director to oversee the day-to-day resident care operations of the Community to ensure the delivery of quality, compassionate care. The RCC supports clinical operations, supervises the care team, coordinates resident services, and promotes resident well-being while ensuring compliance with company policies, regulatory requirements Essential Functions Lead, coach, and support caregivers, Med Techs, CNAs, LPNs, and other care team members to foster accountability, teamwork, and exceptional service. Ensure resident assessments, service plans, and clinical documentation are completed accurately and on time. Assist with resident admissions, pre-admission assessments, care planning, and successful resident transitions. Monitor resident health, communicate changes in condition, and coordinate care with families, physicians, hospice, therapy providers, and other healthcare partners. Build strong relationships with residents and families by providing compassionate, timely communication and addressing care concerns. Assist with staffing, scheduling, and shift coverage while providing hands-on resident care as needed. Coordinate physician appointments, ancillary services, and follow-up care to support resident well-being. Promote person-centered care by modeling the company's Positive Approach to Care philosophy. Support quality assurance initiatives, regulatory surveys, audits, incident follow-up, and continuous improvement efforts. Partner with the leadership team to enhance resident satisfaction, occupancy, and retention. Demonstrate Character Leadership through professionalism, integrity, accountability, teamwork, and a commitment to continuous improvement. Participate in the community's on-call rotation. Perform other duties as assigned.
Required Skills and Experience:
High School Diploma or equivalent required. Associate or Bachelor's Degree strongly preferred in a field related to this position. Must have prior supervisory/management experience. Minimum three years' experience in assisted living or memory care preferred. Experience with Microsoft Word and Excel preferred. Excellent verbal and written communication skills. Able to communicate verbally in person and over the phone. Med Tech certification
