• That you are comfortable making in person visits to complete initial/monthly and as needed psychosocial, emotional, spiritual and bereavement assessments.
• That you are comfortable creating psychosocial, spiritual and bereavement care plans.
• That you are available for Bi-Weekly IDG(Interdisciplinary Group Meeting)meetings.
• That you are comfortable documenting patient visits and phone calls into our EMR
• That you have a current valid driver's license
• That you have reliable transportation
• That you function as advocate for the patient and family unit.
• That you are resourceful as it relates to coordinating patient placements to SNF , Assisted living communities etc.

Responsibilities:

• Provide psychotherapy and counseling services to patients and their families in a hospice setting
• Conduct assessments and develop individualized care plans
• Collaborate with interdisciplinary team members to coordinate patient care
• Utilize motivational interviewing techniques to support patients in achieving their goals
• Collect and analyze data to evaluate patient outcomes and inform treatment plans
• Maintain accurate and timely medical documentation in accordance with regulatory standards
• Provide education and support to patients and families regarding end-of-life care options
• Advocate for patients' rights and ensure their emotional, physical, and spiritual needs are met

Skills:

• Knowledge of medical terminology related to hospice care and end-of-life services
• Experience working with pediatric patients and their families in a healthcare setting
• Strong assessment and data collection skills to inform treatment planning
• Ability to provide compassionate care to seniors and individuals with complex medical needs
• Excellent communication and interpersonal skills to effectively collaborate with interdisciplinary team members
• Familiarity with psychotherapy techniques for addressing grief, loss, and emotional distress
• Attention to detail and strong organizational skills for maintaining accurate medical documentation
