Admissions & Resident Care Coordinator Pleasant View Care Center - Whiting, IA Full-Time This position is well-suited for individuals with experience as a caseworker, LPN, office manager, social worker, or healthcare professional who is passionate about resident advocacy and family support.

Primary Responsibilities:

Assist with facility tours, admissions, discharges and care planning processes Work with insurance companies for pre-authorization for admissions Work with Medicaid applications Act as a liaison between residents, families, and the care team Coordinate services with hospitals, providers, and community agencies Address psychosocial needs of residents and provide emotional support and conflict management to residents experiencing behavioral or emotional challenges related to dementia or other conditions. Maintain accurate records & documentation in Electronic Medical Records and ensure regulatory compliance. Be able to work with time sensitive requirements Collaborate with interdisciplinary team members and leadership

Qualifications:

Prior experience in assisted living facilities, nursing homes, or home care environments is highly preferred Strong communication, organizational , and problem-solving skills Ability to work independently and manage multiple priorities Why Join Pleasant View?
