Case Manager

Job Description

JOB RESPONSIBILITIES
Assess consumers for Older Americans Act, Senior Care Act, and other agency programs. Send completed assessments for data entry within five working days and distribute required documents to the appropriate providers. Complete assigned CARE assessments and return assessments to CARE coordinator within two working days of completion. Maintain records, enter data, and complete accurate activity case logs in database required for the program(s). Advocate for the consumer by communicating the needs of the individual and or making referrals to various agencies, providers, and other parties. Complete follow-up regarding the delivery of those services. Provide information and assistance to consumers regarding available long-term care services and supports. Complete required in-person and/or phone contacts with consumers per SCA/OAA policy to review and address the physical, psychological, and social needs of the consumer. Monitor, track, and adjust changes in consumer condition and needs; revise plan of care as needed. Explain options to consumers, their families, and other parties in reaching goals of the consumer. Track and complete annual assessments of consumers in a timely manner. Assist consumers with obtaining paperwork required to become eligible or maintain their eligibility for the Medicaid Home and Community Based Services (HCBS) program. Attend meetings, as necessary. Promote resource development by assisting in development of local sources for providers and services. Strive to challenge the formal service system to be more flexible and accommodating to the consumer's values and preferences. Expand the range of service options by developing new or non-traditional resources and services to meet consumer needs/goals. Completion of other duties as assigned.
MINIMUM QUALIFICATIONS
4-year degree from an accredited college or university in gerontology, nursing, health, social work, human development or related area; or Registered Nurse licensed to practice in the State of Kansas. Experience as a case manager is preferred Excellent written and oral communication skills Excellent organizational skills, including the ability to multi-task and perform tasks at all levels Good computer skills Knowledge of and sensitivity to aging and disability issues Ability to work well with people of all backgrounds, incomes, ages, races and disabilities Good record keeping skills Valid driver's license and reliable transportation; travel required
