The CCBHC intake staff member acts as the first point of contact for potential clients, conducting initial assessments to gather necessary information, determining eligibility for services, and facilitating the admission process by assigning clients to appropriate clinicians within the clinic, all while adhering to CCBHC guidelines and ensuring timely access to care. The primary responsibility is to efficiently screen and triage new clients, ensuring timely and efficient access based on initial presentation and stated need. Responsibilities will include brief assessment and therapeutic interventions to determine the level and type of services needed across the CCBHC's core services. Intake Staff will provide clinical evidence-based services with a focus on integrated mental health, physical health, and substance use recovery.
RESPONSIBILITIES AND TYPICAL WORK ACTIVITIES
Conducts a triage of requests for initial appointments via phone, walk-ins and referrals from other agencies using a trauma informed approach. Facilitate timely initial assessment to individuals who present for same day service. Triage will include brief assessments for mental wellness, substance use, physical health conditions, suicidality, risk of violence, a brief lethality assessment, description of the presenting problem, and level of determination. Identifies strengths and needs of clients and families and acts to involve them in planning for desired services. Provides crisis intervention and resource planning to adults, children and their families as needed. Escalates emergent situations to lead clinician. Works with team members, community partners and service organizations to closely collaborate care and other community services or resources. Works cooperatively with referral sources to ensure continuity of care. Educates the client about the treatment available and next steps in treatment engagement. Verifies insurance coverage, obtains necessary authorizations, consents and referrals, and ensures financial support is in place for the initial visit. Completes timely and accurate documentation of demographic and clinical services in the medical record.
PREFERRED QUALIFICATIONS
Graduation from a regionally accredited or NYS recognized college or university with a bachelor's degree in social work, psychology, human services or a related field preferred. Ability to be diplomatic and professional while exercising excellent judgement and discretion as appropriate. Experience in conducting mental health assessments and/or crisis intervention. Exceptional rapport building skills with people in crisis. Strong communication and interpersonal skills to build rapport with clients. Ability to work independently and as part of a multidisciplinary team. Proficiency with electronic health records and database management
Work Environment:
This is a professional, fulltime (35 hours/week) position in office in our Binghamton, Cortland or Oneida clinic so applicants from those areas are encouraged to apply. There is occasional travel required including to other FCCS clinic sites for client, partner and /or network meetings as well as occasional work outside of established normal business hours to accommodate client need. Applicants should demonstrate an awareness and understanding of LGBTQ issues, and practice gender-affirming interactions with others. MUST maintain HIPAA compliance at all times. May assume additional responsibilities as required.
