It is about knowing the residents you serve, understanding what matters to their families, recognizing when something has changed, and making sure the right people are brought together to solve problems. We are seeking an experienced Case Manager who can become a trusted resource for our residents, families, and interdisciplinary team. The right candidate will bring one of two strong backgrounds: professional social work experience, ideally as an LSW or LMSW, or direct resident case-management experience within an assisted living community. If your strength is building relationships, navigating difficult conversations, coordinating services, advocating for residents, and bringing order to situations where several priorities are competing at once, we would like to meet you. What You'll Do As a Case Manager, you will manage and coordinate the non-clinical aspects of resident care and serve as an important connection between residents, families, community leadership, clinical staff, outside providers, and other resources.

Your responsibilities will include:

Develop strong, professional relationships with residents and their families and serve as a consistent point of contact throughout the resident's stay. Meet with residents regularly to understand concerns, changing needs, preferences, and quality-of-life issues. Communicate with families regarding resident needs, services, concerns, and appropriate follow-up. Coordinate resident services and collaborate with nursing, wellness, home care, administration, benefits, admissions, and outside providers. Advocate for residents while balancing resident choice, safety, regulatory requirements, and the responsibilities of the community. Assist residents and families in navigating benefits, entitlements, Medicaid-related matters, community resources, appointments, and other services as appropriate. Identify barriers affecting a resident's well-being and coordinate resources or referrals to address them. Participate in interdisciplinary meetings and communicate meaningful changes or concerns to the appropriate team members. Maintain accurate, timely, organized case-management documentation. Follow up on outstanding resident and family concerns through resolution rather than simply referring the matter elsewhere. Assist with transitions, hospitalizations, returns to the community, changes in services, and other significant resident events. Maintain resident confidentiality and comply with all applicable organizational, state, federal, and regulatory requirements. Build collaborative relationships across departments while maintaining clear professional boundaries. Manage multiple competing priorities without losing sight of the resident or allowing important follow-up to fall through the cracks. Who We're Looking For We are particularly interested in candidates with either : A social-work background — including candidates who hold an LSW or LMSW , with experience supporting older adults, individuals with disabilities, healthcare populations, or other vulnerable populations; OR Direct assisted-living case-management experience — specifically working with residents and families in an assisted living, adult care, senior living, or comparable residential setting. A bachelor's degree in social work, human services, psychology, healthcare administration, or a related discipline is preferred. Relevant experience may be considered in lieu of a specific degree depending upon the candidate's background. Successful Case Managers at Amber Court are compassionate without losing objectivity, organized without becoming rigid, and comfortable having conversations that are sometimes difficult. You should be someone who can sit with an upset resident and make them feel heard, speak professionally with a concerned family member, collaborate with clinical staff, meet a documentation deadline, and know which issue cannot wait until tomorrow.
