As a Wound Care Technician , you will play a vital role in ensuring the seamless delivery of healthcare services to our patients. Your attention to detail, excellent communication skills, and proactive approach will contribute to the overall quality of patient care. This position requires travel throughout the assigned territory.
Responsibilities Appointment Coordination:
Contact patients one day before their scheduled appointment to confirm availability and identify any scheduling conflicts. Ensure appointments align with each patient's needs and availability.
Communication Management:
While en route to a patient's home, contact the patient or caregiver to provide an estimated arrival time and address any immediate questions or concerns.
Appointment Scheduling:
Coordinate weekly appointments through Wound Expert for providers, ensuring timely and effective patient consultations.
Appointment Updates:
Promptly notify both the patient and the home health agency of any changes to a provider's scheduled appointment day.
Supply Oversight:
Confirm that providers have the necessary supplies for scheduled appointments to support efficient and uninterrupted patient care.
Data Documentation:
Record wound measurements and capture photographs using the Wound Expert platform, ensuring accurate and comprehensive documentation of each patient's condition.
Patient Information Accuracy:
Verify that all patient information collected during the initial visit—including insurance information, pharmacy details, medication lists, and allergies—is accurate and up to date.
Insurance Verification:
Confirm during each visit that the patient's insurance information has not changed to help prevent discrepancies that could affect coverage or services.
Referral Assistance:
Assist providers with sending required faxes and referrals, including home health referrals, clinic referrals, and other relevant documentation. Involve the Intake Department only when a referral requires prior authorization related to diagnostic studies.
Wound Vac Support:
For patients utilizing wound vac therapy, obtain accurate wound measurements every two weeks and fax the required information to the designated Durable Medical Equipment (DME) company responsible for supplying the wound vac. Your commitment to precision, organization, effective communication, and compassionate patient care will contribute significantly to Strategic Healthcare Solutions' mission of delivering high-quality healthcare services.
