Job Description

The primary role of the Vibrance Health Community Health Worker is to focus on educational opportunities, promote Vibrance Health services and programs, and perform case management. CHW is the key resource for referral services with follow-up, team huddles, and counseling.
Key Duties of Community Health Worker:
Develops a strong relationship and networks with the communities Vibrance Health serves. Provides follow-up care and assists patients with Vibrance Health to receive appropriate services. Serves as role model and mentor in group or individual meetings. Communicates effectively with other members of the clinical staff. Performs other related duties as assigned. Adheres to the policies and procedures of Vibrance Health. Participates in efforts to achieve and maintain PCMH (Patient-Centered Medical Home). Promote social services, complete referrals, promote outreach, and educate patients. Promotes and participates in care team. Assists patients and staff with patient referrals and follow-up. Assists with removal of barriers to health care. Works in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patient's health problems. Network with other agencies and utilize community resources to assist patient needs. Provide community outreach, networking, and liaison with community programs as needed. Educates patients regarding community resources and assists with application processes. Maintains patient privacy in compliance with HIPAA federal guidelines and Vibrance Health policies. Helps in other areas assisting the physicians, when needed, in continuing the flow of the patients. Communicates with patients in a caring, professional manner. Culturally sensitive in the use of language, practices, and beliefs. Possesses the ability to master information, plan and lead classes and collect and use information from community people. Possesses good organizational skills and is self-directed. Ability to communicate and relate to people at all levels in the organization. Ability to respond effectively to inquiries about Vibrance Health's services. Demonstrates flexibility and emotional control in handling difficult or stressful situation Bilingual is a plus Participates in community outreach events & assists community members in accessing health related & community resources Assists with Safe Riders program, teaching classes, enrolling caregivers, reporting & inventory maintenance Trains newly hired Pediatric Referral Coordinators, continually mentors team members regarding referral best practices Shares records for annual UDS reporting. Maintains referral source contacts & updates on process changes
Pay:
$17.00 - $20.00 per hour
