Population Health Coordinator Premier Family Medical
•Lindon, UT Full-Time | Monday-Friday | $17.00
•19.00/hour DOE Premier Family Medical is looking for a Population Health Coordinator to join our Quality and Population Health team. This position supports our efforts to improve patient outcomes and quality of care through patient outreach, quality data management, insurance initiatives, and coordination with our clinical teams. This is a great opportunity for someone with Medical Assistant experience who is interested in moving into the administrative, quality, and population health side of healthcare. On-the-job training is provided. Schedule & Hybrid Opportunity This is a full-time, Monday-Friday position based out of our Lindon office. A hybrid work schedule may be available after the first 3 months of in-office training and demonstrated proficiency in the position. Once approved for hybrid work, the typical schedule includes: 2 days per week in the Lindon office, including Thursdays 3 days per week working remotely This is not a fully remote position. Employees must be available to work in the office weekly, and additional in-office time may be required based on operational needs, particularly during the 4th quarter. This is a permanent full-time position. We are unable to accommodate schedules that change frequently due to school or other outside commitments. What You'll Do Conduct pre-visit planning for upcoming Medicare Wellness Visits Perform patient outreach to schedule, confirm, and coordinate appropriate care Assist with Emergency Department (ED) patient follow-ups Support Transitional Care Management (TCM), including tracking patients and coordinating follow-up care Research and manage patient data and documentation related to quality measures Identify and track gaps in care and help obtain missing information needed to close quality measures Maintain and reconcile patient rosters and gaps in care for multiple insurance providers Navigate insurance and payer portals to retrieve and verify patient information Reconcile and submit HCC forms and other required documentation to insurance companies Complete quality-related data retrieval and projects for various insurance contracts Assist with insurance contract tracking and compliance documentation Upload forms and supporting documentation into electronic patient records Participate in payer meetings and assist with insurance quality and incentive initiatives Collaborate with clinical teams, insurance companies, and other departments Maintain patient confidentiality and comply with HIPAA requirements Adapt to changing CMS, payer, and quality program requirements Complete additional Population Health and Quality projects as assigned Qualifications Minimum 6 months of Medical Assistant experience required High school diploma or GED required Healthcare, clinical, or medical administrative experience preferred Familiarity with HEDIS, HCC, gaps in care, value-based care, or insurance quality measures is a plus Strong computer skills, including Microsoft Excel Comfortable learning and navigating EMR systems, insurance portals, spreadsheets, and other healthcare software Excellent verbal and written communication skills Strong attention to detail and organizational skills Ability to manage multiple tasks, projects, and deadlines Self-motivated and comfortable working independently Professional and comfortable communicating with patients, providers, insurance companies, and other healthcare partners Collaborative, dependable, and willing to help wherever needed Benefits On-the-job training Medical, Dental, Vision, Life Insurance, HSA & FSA Premier Family Medical pays 70% of the employee's monthly insurance premium 7 Paid Holidays Almost 3 weeks of Paid Time Off 401(k) + Matching Quarterly Bonuses Scrubs/Swag
