Overview The Practice Transformation Specialist (PTS) provides operational coordination, project management, and data analytics support to healthcare organizations to improve clinical workflows, strengthen quality outcomes, and support the transition to value-based care delivery models. This role collaborates with clinic teams to enhance access to health services for identified patient populations while providing a liaison role that improves patient and provider engagement, care coordination, and overall practice performance.
Responsibilities:
Build relationships with clinic staff to implement process changes and educate on efficient use of EHR's and workflows. Increase access to health services with/for an identified population through the health centers' population team. Develop, maintain, and explain analytical reports and data tools to support data understanding and informed decision-making across teams. Lead practice transformation and practice performance improvement initiatives. Guide Health Centers in achieving targeted goals and outcomes, to improve quality, efficiency, and utilization. Collaborate with clinic team, health plans, and data analytics teams to identify improvement. Analyze data to identify trends, prescribing patterns, and opportunities for improvement, and provide meaningful insights to support operational and clinical decision-making. Collaborate with the clinic team to strengthen chronic disease management and support preventive care initiatives aimed at improving patient outcomes and reducing unnecessary use of emergency and other high-cost care settings. Develop transformation plan, including strategies for improvement in utilization metrics and HEDIS and other Quality Measures, based on performance analysis data and established targets. Facilitate meetings with Clinic Management team to monitor, present, and discuss progress on the transformation plan. Analyze cost, utilization, and quality data from multiple data sources, including, but not limited to, payer portals and payer claims. Establish relationships, gather input, and facilitate communication amongst key partners. Review monthly practice team reports and facilitate improvement strategies.
Qualifications and Skills:
Bachelor's degreein health care or related field or equivalent health plan. Previous experience in primary care settings, project management, or equivalent experience. Strong communication, interpersonal skills, and proficiency in analyzing data to identify trends. Understanding of Value-based and quality measures. Must be proficient in Excel, Word, PowerPoint, Outlook, and Zoom. Electronic Medical Records (EMR) experience. Ability to travel
Pay:
$70,000.00 - $100,000.00 per year
