We are seeking a dedicated and compassionate MDS professional to join our team at Avir at Pebble Creek in El Paso, TX. Conduct thorough and accurate Minimum Data Set (MDS) assessments for residents, identifying their individual needs and identifying areas for improvement Develop and implement comprehensive care plans, collaborating with interdisciplinary teams to ensure resident-centered care Utilize MDS data to identify trends and areas for quality improvement, contributing to the development of effective care strategies Provide education and training to staff on MDS processes and best practices Maintain accurate and up-to-date records of resident assessments, care plans, and progress About the
