Substance Use Treatment Referral Services Case Manager Position Summary:
The Substance Use Treatment Referral Services Case Manager is responsible for coordinating care and connecting individuals with appropriate substance use treatment services. This role focuses on client advocacy, assessment, and ongoing support to ensure successful linkage to treatment and recovery resources, particularly for justice-involved and vulnerable populations.
Key Responsibilities:
Attend and actively participate in drug court proceedings, providing updates and support for referred clients
Conduct comprehensive client intakes, including screening for substance use, mental health needs, and social determinants of health
Perform weekly outreach visits to local homeless encampments to engage individuals and connect them with services
Coordinate with treatment facilities to facilitate timely referrals and placement, including securing bed availability for incarcerated individuals upon release
Maintain regular follow-up communication with treatment providers to monitor referral outcomes and client admission status
Conduct post-assessment follow-ups with clients to ensure contact with recommended treatment facilities is established
Provide clients with community resources, including housing, employment, healthcare, and recovery support services
Complete Human Needs Assessments to identify client strengths, barriers, and service needs
Advocate for clients to reduce barriers to treatment access and continuity of care
Maintain accurate and timely documentation in accordance with organizational and funding requirements
Participate in community outreach events to promote services and engage with the community
Qualifications:
Bachelor's degree in social work, psychology, or a related field
Experience working with individuals with substance use disorders, justice-involved populations, or individuals experiencing homelessness
Knowledge of substance use treatment modalities, recovery resources, and community-based services
Strong communication, organizational, and case management skills
Ability to build rapport with diverse populations and maintain professional boundaries
Experience collaborating with courts, correctional facilities, and treatment providers preferred
Must have, or have the ability to obtain, ADMH Office of Substance Use Treatment Services (OSUTS) Case Management Certification
If in recovery, must have a demonstrated minimum of two (2) years of sobriety
Must be able to pass a pre-employment drug screening

Work Schedule & Environment:
Monday through Friday, 9:00 AM - 5:00 PM
Must be willing to work occasional Saturdays as needed
Must be willing to attend community outreach events

$43,680.00 - $49,920.00 per year
