Case Manager - Lynwood

Job Description

A Case Manager provides case management and behavioral guidance for program youth. Persons employed as case managers are responsible for behavioral case management and linkage services for the youth and families being served as well as skill building for individual rehab services. The position involves working designated shifts under the direct supervision of the Clinical Supervisor or Program Director.
DUTIES:
1.
CASE MANAGEMENT
Provides general case management and skill building for client's and their and families in accordance with the policies of the program and referring/regulatory agencies (DMH, DCFS, Probation, schools). 2.
OBJECTIVES
Develops measurable case management and individual rehab objectives for clients as outlined in the Client Care Plan and monitors progress toward those goals. 3.
PRODUCTIVITY
Completes thorough and timely documentation of all contacts with clients and maintains productivity standards set forth by the agency Tier System and internal policies. 4.
MEETINGS
Attends all staff meetings, in-service training and conferences as required and scheduled. 5.
CONSULTATION
Regularly consults with minor's parent or guardian, probation officer or CSW, and school staff. 6.
TREATMENT TEAM
Meets regularly with other members of the treatment team concerning client and program issues and coordinates needed services and linkages. 7.
CONFIDENTIALITY
Maintains strict confidentiality over all records and information relative to minors. 8.
RESPECT:
Demonstrates respect for the intrinsic value of each person, their individual differences and culture. 9.
CRISIS:
Ensures the safety of youth. Provides direct crisis intervention and therapeutic guidance. 10.
CLIENT INTEREST
Stimulates client's interests in various clinical offerings. Monitors progress and maintains close contact with client and family. 11.
BOUNDARIES
Establishes positive, respectful and nurturing relationships with clients and families within professional boundaries and in accordance with agency policies and procedures. 12.
TRANSLATION
Provides translation services as needed. 13.
STRENGTH BASED
Is capable of accurately identifying and building on the strengths of clients to better reach their potential. 14.
TRAVEL:
Travels independently to various environments (buildings with stairs, various communities, schools, etc.) to provide services as needed. 15.
OTHER:
Performs other duties as assigned by supervisor.
QUALIFICATIONS
1. Bachelor's degree related to the fields of psychology, social work or other majors defined by the Department of Mental Health as a related degree. 2. Valid California Drivers License, good driving record, and must be able to drive to fulfill the duties of the position.
