licensed primary mental health & addiction treatment provider in the South Florida metro area. We work closely with the surrounding community to provide accessible addiction and mental health treatment for all individuals. We are currently seeking a Case Manager to join our team! The ideal candidate will provide case management services (e.g., information, referral, and linkage). CM will facilitate client care by identifying and assessing client needs and evaluate, coordinate, and oversee necessary services and/or treatment is provided. Also, provides support and assistance to clinical department in various activities. Works with and in consultation with licensed clinical and medical staff as part of multi-disciplinary team.

Responsibilities:

Responsible for case management duties: engagement, assessment, service planning, development, coordination, linkage, monitoring and accompanying documentation. Responsible for completing intake process with newly admitted clients, as well as any required screens or assessments, as appropriate (may be completed by alternate clinical staff). Responsible for coordinating a patients discharge and aftercare activities, working in collaboration with family and other healthcare providers and facilities. Reviews patients' employment status to assess the need for FMLA or short/long term disability applications and facilitate completion/submission of such documentation. Responsible for assisting and educating patients with necessary paperwork for Unemployment, COBRA, food stamps, etc. and follow-up process. Responsible for coordinating services being provided, arranging resources, including transportation, if necessary. Responsible for reviewing patients' legal status to assess the need for intervention on their behalf, including contacting attorneys and/or providing documentation that verifies a patient's presence in treatment, postponing court dates, filing appropriate paperwork and documentation, or facilitating remote attendance if appropriate and necessary. Coach patients on how to appropriately communicate their presence in treatment to employers, family members, and legal authorities; and facilitate such communications at patient request. Responsible for coordinating aftercare services, arranging resources, including transportation, if necessary. (i.e., PCP, outpatient services, or private therapy, etc.) Practices and maintains confidentiality of patient information and facility activities, adhering to relevant HIPAA and 42 CFR Part 2 regulations. Exercises professional boundaries while adhering to ethical standards and code of conduct.

Position Requirements :

Graduate from an accredited college or university with a minimum of bachelor's degree in psychology, social work, counseling, or another health and/or human services related field. Proficient at Microsoft Office365 Suite, preferred. Knowledge of

is a plus but will train. Must be able to pass an AHCA Level II Background Screening.

Experience :

A minimum of 1 year experience in a mental health/psychiatric setting. At least two (2) years' experience in case management, preferred.
