Position Summary:

We are seeking a dedicated and compassionate Case Manager to join our team! The Case Manager will play a key role in supporting individuals in their recovery journey by assisting with case coordination, facilitating access to services, and providing ongoing support throughout the treatment process. This position will involve working closely with individuals struggling with substance use disorders, ensuring they receive the necessary resources and services to promote recovery and overall well-being. The successful candidate will be committed to empowering clients to achieve their recovery goals and will work in collaboration with a multidisciplinary team to provide comprehensive care.

Duties and Responsibilities:

Conduct thorough assessments to identify clients' physical, emotional, and social needs, and determine the most appropriate services to address them. Coordinate client services and ensure access to appropriate community resources such as housing, employment assistance, transportation, and healthcare. Act as the primary point of contact for clients, providing ongoing case management, support, and follow-up throughout their treatment. Conduct regular follow-ups with clients to ensure they are receiving appropriate services and making progress toward treatment goals. Monitor and track clients' progress, documenting significant changes, challenges, and successes in accordance with facility policies and confidentiality standards (HIPAA). Collaborate with therapists, counselors, medical professionals, and other team members to create and modify treatment plans as needed. Advocate for clients by assisting with navigating the healthcare system, securing resources, and addressing barriers to recovery. Provide crisis intervention and support as necessary, ensuring clients receive appropriate care during emergencies. Educate clients and families about available community resources and recovery support options. Ensure that all services provided meet the ethical standards and regulations of the facility and state laws. Attend and actively participate in case conferences, team meetings, and supervision sessions to contribute to comprehensive client care.

Qualifications:

Bachelor's degree in Social Work, Psychology, Human Services, or related field preferred. Certification in substance abuse counseling preferred.

Experience:

Two years of experience in case management, social services or mental health/substance abuse setting, preferred. Knowledge of substance abuse recovery models, community resources, and social service systems.

Experience working with individuals in recovery and those with co-occurring mental health disorders is a plus. Proven ability to maintain client confidentiality and adhere to ethical and professional standards. Skills, Knowledge, and Abilities (Competencies): Strong understanding of case management principles, including the ability to assess, plan, and coordinate care for clients with substance use disorders. In-depth knowledge of substance use disorders, treatment modalities, recovery processes, and the challenges faced by individuals in recovery. Ability to provide care that is respectful of and responsive to the cultural and socioeconomic diversity of clients, ensuring that services are tailored to meet individual needs. Excellent verbal and written communication skills, including the ability to build rapport with clients, facilitate group discussions, and document client progress accurately. Strong advocacy skills, with the ability to connect clients to community resources, support services, and external agencies. Ability to identify challenges and creatively address client needs, including overcoming barriers to treatment and recovery. Strong crisis intervention skills to effectively address emergencies, de-escalate situations, and provide immediate support to clients. High ethical standards, with a commitment to client confidentiality, adherence to legal and regulatory requirements, and maintaining professional boundaries. Ability to work collaboratively in a multidisciplinary team, ensuring communication and coordination of care across all aspects of treatment. Strong organizational and time management skills, with the ability to manage a caseload, prioritize tasks, and meet deadlines in a fast-paced environment. Focus on empowering clients in their recovery journey, promoting their independence, self-sufficiency, and long-term success. Familiarity with the legal and ethical guidelines surrounding substance abuse treatment, including HIPAA, confidentiality, and reporting requirements.

Pay:

$18.00 - $22.00 per hour
