Our practice is dedicated to evaluating and treating acute pain associated with soft tissue injuries. We are professional, challenging and collaborative. The Case Manager is responsible for coordinating patient care throughout the treatment process while serving as a primary point of communication between patients, healthcare providers, attorneys, and referral partners. This position requires strong organization, communication, and attention to detail to ensure patients progress through their recommended treatment plans and that all case-related documentation remains accurate and current. The ideal candidate is compassionate, proactive, highly organized, and comfortable managing multiple patient cases and competing priorities in a fast-paced healthcare environment. Essential Duties & Responsibilities Manage an assigned caseload and maintain consistent communication with patients throughout their course of care. Complete new patient preparation and review patient charts for accuracy and completeness. Conduct introductory and follow-up calls with patients and attorneys to provide guidance regarding appointments, referrals, and next steps in treatment. Coordinate referrals, imaging, specialist evaluations, and other services ordered by providers. Track patient appointments and treatment progression and follow up on missed or incomplete care. Maintain accurate and timely documentation within eClinicalWorks (ECW), ChiroTouch, patient trackers, and other case-management systems. Monitor daily task queues, telephone encounters, emails, digital communications, and assigned case-management tasks. Maintain and update patient demographic information, attorney information, imaging, provider documentation, referrals, and other relevant case documents. Maintain accurate case alerts, including the assigned Case Manager, date of loss, treatment-to-date, liability status, policy-limit requests, and available coverage information when applicable. Communicate professionally with attorney offices regarding patient treatment status, Letters of Protection, procedure estimates, liability information, policy limits, and other case-related needs. Upload, organize, and maintain records and supporting documentation within appropriate systems and portals, including MoveDocs. Ensure referrals are processed promptly and follow up with patients and outside facilities to prevent unnecessary delays in care. Coordinate with providers, clinical staff, Intake, imaging facilities, and other departments to resolve patient or case-related concerns. Protect patient confidentiality and comply with HIPAA requirements and organizational policies. Escalate clinical, operational, patient-service, or case concerns appropriately. Participate in team meetings, training, workflow improvement initiatives, and departmental quality-assurance activities. A self starter that anticipates patient and provider needs. Familiar with eClinical Works and ChiroTouch EHR systems.
