Flexible Schedule Description The goal of the Case Manager/CBRS Manager is to work with individuals and families to help them identify and link up to community resources to increase stability and wellness so they can live as independently as possible in their communities.
CBRS Managers coordinate services and advocate for the person served when they are not able to advocate for themselves. Case Manager/CBRS Managers can assist with the following: Maintaining eligibility, filling out forms for services in the community, linking them to housing and financial services, Vocational and educational resources, Social and interpersonal supports, and health and medical supports as needed. Qualifications
Must possess a bachelor's degree or higher in relevant Social Services field
Current valid driver's license
Maintain Car Insurance
Maintain Professional Liability Insurance Coverage
Current Enhanced Background Check through Health and Welfare Duties
Organize, direct and assist in the delivery of Support Services .
Communicate and work cooperatively with clients, families of clients and co-workers.
Must possess computer skills to include word processing, excel and database experience.
Ability to collaborate with current Mental Health team, attend mandatory meetings, report timely/weekly progress notes, and reviews as needed.
Assess client's needs and develop a CM Treatment Plan for each client.
Must be able to Observe all agency policies and procedures.
