Job Description

Abbey Placements is looking for a Case Manager for individuals with intellectual disabilities (QIDP) for a non-profit organization in the Peoria, IL area. Description of Case Manager (QIDP) Develop behavior plan for individuals who require behavioral support In conjunction with the HR department, interview, hire, supervise, assign, evaluate the performance of, and when necessary, discharge assigned personnel directly and in cooperation with appropriate department heads Complete monthly summaries and ensure all documentation is completed to meet clinical and financial requirements Develop personal plans for all people on caseload, including coordinating and facilitating ISP annual, along with other meetings as appropriate Maintain positive parent and guardian relations for individuals on the team Full-time, direct hire position Requirements of Case Manager (QIDP) Bachelor's degree in human services or education One year experience with persons with developmental disabilities preferred Valid driver's license and proof of insurance Must pass a drug screen and a criminal background check Reliable work history with good references required Compensation of Case Manager (QIDP) $50K
