The Case Manager coordinates referrals, service planning, and client documentation for designated caseload(s).

13 weeks to start, with the likelihood of extension

Setting:

Psychiatric/Behavioral Schedule:

Monday - Friday, 8am to 4pm, with a standing 12pm-8pm shift. Includes rotating weekends

Salary:

$28/hr

Position Description:

Provide case management services as part of a multidisciplinary team Develop treatment plans with consumers Act as a primary case manager to consumers in accordance with Eliot's policies and PACT standards Participate in completing assessments as assigned by the Team Leader or Lead Clinician in a timely manner Assess clients' functions, strengths, and needs Develop therapeutic connections with each client and provide clinical interventions Provide problem-solving supports and interventions to clients Caseload is 5-8 clients

Qualifications:

Valid driver's license Reliable vehicle, as you will be asked to take clients out into the community 1+ years of prior experience working with adults having severe and persistent mental illness Please feel free to contact me with any questions you may have regarding the position.
