Position Description:

Responsibilities and Duties The Case Manager is responsible for: Programming
The overall development and implementation of all Individual(s) programs related to his/her supported living services
Upholding and ensuring compliance with all local, state and federal rules and regulations along with all agency policies and procedures
Responsible for the Intake, Discharge or Transfer documentation
Be the main point-of-contact with County Boards, Individuals and Guardians
Organize and participate in Individual planning and monthly Improvement meetings
Responsible for compiling, organizing and participating in all audits, compliance reviews and other inspections as they occur Medical Coordination
Ensure all medical, dental, psychiatric and other health related appointments for the Individual are schedule, attended and maintained in the log of medical appointments
Delegate staff to attend health related appointments with Individuals and ensure all documentation is complete
Responsible for all follow-up recommendation including any additional appointments, medication changes, charting, filing and documentation related to any appointment or health care provision
Sign/check in new medications, treatments and other health related equipment as ordered and ensure MARs are appropriate to all physician orders
Provide written summaries regarding current medication, instructions on delivery of medication and other required documentation for all health/physician related issues
Prepare and review all medical and delegated nursing documentation required for any compliance reviews in a manner easily reviewed by auditors
What We're Looking For:
Must be at least 21 years of age Must have a HS diploma/GED; Bachelor's degree preferred but not required Must have less than 6 points on current driving record Must be able to pass background check Must be able to pass a pre-employment drug screen Must have good verbal and written communication skills
