Job Description

HIV/AIDS MEDICAL CASE MANAGER
Mission:
To provide case management and supportive services to persons living with
HIV/AIDS
Responsibilities:
Primary Client Care Functions 1. Provides case management services to clients living with
HIV/AIDS
(Optimum caseload size: 30-35 clients; contingent contract requirements, standards of care, and clients' needs.) a. Contact new clients as assigned in a timely manner b. Assessment of new clients c. Development of an individualized service care plan d. Identification of resources which would be appropriate for and beneficial to clients e. Referral to services appropriate for and beneficial to clients f. Assist clients in preparing and documenting applications for relevant and beneficial services. g. Accompany clients to medical and social service appointments as needed h. Educate clients in areas of
HIV/AIDS
prevention, secondary risk reduction and strategies for healthy living. Provide emotional support for clients i. Empower clients toward self-confidence and independence. j. Advocate for individual clients as appropriate. k. Display a working knowledge and ability to use the AACO database system (Careware) and ensure that documentation is current. l. Ensure Service Units are documented in Careware at 480 units for your caseload based upon a caseload of 35 clients. 2. Provides linkage to supportive services including transportation, housing, nutritional, and emergency financial assistance Primary Administrative Functions Complete required paperwork in a timely and accurate manner. Internal Agency Administration 1. Meeting regularly with assigned supervisor for support, direction, supervision, and feedback 2. Communicating and collaborating with other Agency staff in order to increase the Agency's effectiveness. 3. Adhering to all Agency policies and procedures (e.g., confidentiality). 4. Participating in staff meetings at all levels of the Agency as appropriate or as assigned. 5. Completing and submitting all required, relevant, and assigned paperwork in a timely and accurate manner. 6. Supporting and participating in Agency activities such as staff retreats, special events, volunteer recognition events, and fundraising as appropriate or as assigned. 7. Performing other duties as assigned. Staff Development 1. Seeking and participating in development activities in order to become better able to contribute to effective service provision. 2. Stays current with knowledge of the population of clients receiving services from
HIV/AIDS
program. 3. Stays current with medical knowledge and treatments relevant to
HIV/AIDS
clients receiving services from
HIV/AIDS
program. 4. Stays current with knowledge of resources available to
HIV/AIDS
clients.
Qualifications:
1. Commitment to the Agency's mission and philosophy. 2. Bachelor's degree in Social Work or related human services discipline from an accredited university. Master's degree preferred. 3. Experience providing care to persons living with
HIV/AIDS
preferred. 4. Compassion and acceptance of persons living with
HIV/AIDS. 5.
Valid driver's license. 6. Access to an insured motor vehicle. 7. Basic computer skills including word processing and spreadsheets. 8. Strong interpersonal skills. 9. Detail oriented and organized. 10. Able to work independently on multiple tasks. 11. Meet all required clearances for employment (e.g., IRCA, Criminal Record Check, etc.).

$38,000.00 - $39,000.00 per year
