To provide health care services to high barrier, medically fragile homeless clients living in our permanent supportive housing units &

Supportive Shelter Responsibilities :

Nurse will work with shelter/housing case managers, clients and affiliated organizations in the community to promote the health of clientele. Maintain a client caseload up to 33 clients who stay in StreetLight's Medically Fragile Permanent Supportive Housing units or shelter. Provide health management services to all clients including a health assessment intake on all new residents to ensure admission is appropriate for PSH housing or shelter. Obtain health information, including medical and behavior health history, prescribed medications, and plan of care on clients discharged from medical facilities and rehabilitation centers to ensure continuity of care. Maintain health records on all clients. Assist with client access to health care services, to include establishing a medical home and primary care provider, assist with making appointments as needed, ensure follow up appointments are made and kept. Provide wound care as needed and monitor client response to care. Engage shelter residents frequently to assess the need for intervention and foster an environment that builds trust and relationships. Conduct classes to shelter staff on topics pertinent to their needs, such as handling of client medications, etc. Conduct training for clients on health and medication management. Work with staff to review existing policies and protocols on care of the clients, update and establish new policies and protocols as warranted. Collaborate with staff regarding the continuum of care and the appropriate discharge planning when a client is transitioning to permanent housing. Provide all required documentation/reports in a timely manner, which may include client follow-up, outcome evaluation, client case notes and maintaining client files. Participate in staff meetings, staff training programs, supervisory sessions, and accept the responsibility for aiding the development of positive team relationships as requested. Adhere to agency policy, procedures and the professional code of ethics. Remote work and flex time options are contingent on meeting performance standard after an evaluation period.
