HIV Medical Case Manager

Job Description

Due to the expansion of services and a growing caseload, a HIV case manager position is available in the Charleston area with the AIDS Task Force, a non-profit agency that is a leader in providing quality comprehensive services to HIV infected West Virginians. The AIDS Task Force, established in 1987, provides direct services, including medical case management to West Virginians living with
HIV/AIDS
through the Ryan White Part B Program. The ideal candidate for this full time salaried position will have a minimum of a Bachelors Degree in Social Work, or a current nursing license. Candidates should be energetic, self-motivated, able to work as a team of service providers, have a caring attitude and possess excellent communication, organizational and inter personal skills. All applicants must have a valid driver's license and have dependable transportation. A current WV Social Work license or nursing license is required for the HIV Case Manager position The successful applicant for the HIV Case Manager will perform intakes, assessments and work with HIV infected clients to assist in accessing and maintaining a continuum of care that improves their health outcomes. Duties include coordination of medical treatment, initial and ongoing assessments, treatment adherence counseling, creating and monitoring individual service plans and applying for entitlement programs. Full
