Job Summary We are seeking a dedicated and dynamic Medical Case Manager to join our healthcare team. In this vital role, you will coordinate and oversee patient care plans, ensuring seamless communication among healthcare providers, patients, and support services. Your expertise will help optimize patient outcomes, facilitate discharge planning, and manage complex cases across diverse clinical settings. This position offers an exciting opportunity to make a meaningful difference in patients' lives by providing comprehensive case management within a fast-paced, collaborative environment. Duties Conduct thorough assessments of patients' medical, social, and behavioral health needs to develop personalized care plans. Coordinate with hospitals, clinics, social services, and community resources to ensure appropriate discharge planning and ongoing support. Manage utilization review and utilization management processes to ensure appropriate level of care while adhering to healthcare policies and regulations. Collaborate with multidisciplinary teams including nurses, social workers, physicians, and therapists to facilitate patient-centered care. Document all case management activities accurately within electronic medical record (EMR) or electronic health record (EHR) systems while maintaining compliance with HIPAA regulations. Provide patient education on treatment options, medication management, and available community resources. Assist with intake procedures for new patients and conduct ongoing case reviews to monitor progress and adjust care plans as needed. Skills Extensive knowledge of medical terminology, hospital procedures, and healthcare regulations such as HIPAA. Critical care experience including hospital setting, outpatient clinic, and specialty care. Strong understanding of managed care principles, utilization review, discharge planning, and health insurance processes. Proficiency in EMR/EHR systems along with Microsoft Office Suite for documentation and communication purposes. Excellent client communication skills with the ability to work effectively with individuals from diverse cultural backgrounds. Experience working in medical based clinic, behavioral health, hospice care or acute care settings is a plus. Ability to handle complex cases involving social services coordination, behavioral health issues, or working within Level I/II trauma centers.
