In this vital role, you will provide safe and effective medication management and support to residents across various care settings, including assisted living, nursing homes, hospitals, and home care environments. Your expertise will ensure residents receive personalized care that promotes their well-being, dignity, and comfort. This position offers flexible hours and the opportunity to make a meaningful difference in the lives of individuals with diverse needs. Duties Administer medications accurately according to physician orders while adhering to HIPAA regulations and medical documentation standards Monitor and record vital signs, medication effects, and patient responses using EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems such as Epic or Cerner Assist with activities of daily living, including tube feeding, catheter care, and other personal care tasks in compliance with safety protocols Collaborate with nursing staff and other healthcare professionals to develop and update individualized care plans for residents with dementia, Alzheimer's, or other cognitive impairments Maintain detailed medical records, document medication administration, and ensure compliance with HIPAA privacy standards Support residents during emergencies or acute care situations by providing immediate assistance and communicating effectively with emergency responders Provide compassionate caregiving in various settings such as assisted living facilities, hospitals, hospice environments, or home care visits Skills Proven experience working in assisted living facilities, nursing homes, hospitals, or home care environments Strong knowledge of medical terminology, physiology, anatomy, and medication administration procedures Familiarity with EMR/EHR systems like Epic or Cerner for accurate documentation Experience caring for individuals with developmental disabilities, dementia, Alzheimer's disease, or memory care needs Ability to perform vital signs assessment, catheter care, tube feeding support, and hospice or palliative care tasks Excellent communication skills for collaborating with multidisciplinary teams and providing empathetic resident support Knowledge of HIPAA regulations to ensure confidentiality and secure handling of medical records Join us in delivering exceptional care that enhances the quality of life for those we serve!
