Key Responsibilities :
Book appointments in the Electronic Health Record (EHR) according to provider scope. Use Microsoft Office to communicate and report information. Document patient encounters in EHR systems. Participate in health fairs, outreach events, and tabling to promote services. Maintain logs and follow-up on no-show appointments. Schedule interpreters and transportation via patient health plans. Conduct outreach and enrollment in public benefit programs. Connect patients to internal and external resources to meet social and health needs. Serve as a liaison between the community and health/social services. Build and maintain appropriate relationships with patients, staff, and community partners. Support departmental documentation standards and adhere to clinic policies. Work flexible hours, including evenings and weekends, across multiple locations (clinics, schools, mobile units, homeless encampments, warming centers, patient homes). Secondary Responsibilities Attend workshops, training, and meetings as needed. Perform other duties as assigned by supervisor. "TSHL01"
Pay:
$25.00 - $26.00 per hour
