Community Health Worker (Field-Based) Washington, DC (Hybrid/Field-Based) $24.00

00 per hour Full-Time | Monday-Friday | 8:00 AM-5:00 PM Join Our Team We are seeking an experienced Community Health Worker (CHW) to support high-risk members throughout Washington, DC. This is a highly field-based position focused on connecting members to healthcare services, addressing social determinants of health (SDOH), and helping individuals overcome barriers to care. What You'll Do Community Outreach & Member Engagement Conduct proactive door-to-door outreach and field visits throughout Washington, DC, with a primary focus on Wards 7 and 8 . Locate assigned members and reconnect disengaged individuals with care management services. Build trust and establish strong relationships with members in community settings. Verify member contact information and assist clinical teams with outreach efforts. Address Social Determinants of Health (SDOH) Help members access food, housing, transportation, and other community resources. Deliver or transport essential supplies, documentation, or other needed items. Educate members on available social services and community programs. Support members in navigating local assistance programs. Collaborate with the Care Team Partner closely with Nurse Case Managers and Licensed Clinical Social Workers (LCSWs/LISWs). Provide timely updates on member encounters, environmental concerns, and outreach efforts. Maintain accurate documentation using tablets, mobile devices, and field technology. Daily Operations Begin each day with office check-ins and case preparation before deploying into the field. Follow all organizational policies, confidentiality standards, and safety protocols. Manage your schedule effectively while working independently in the community. Required Qualifications Previous professional experience in: Community Health Worker Community Outreach Care Coordination Case Management Support Managed Care Organization (MCO) Community Navigation Entry-level applicants without field outreach experience will not be considered. Valid driver's license with a reliable personal vehicle. Clean driving record. Strong knowledge of Washington, DC neighborhoods, especially Wards 7 and 8 . Comfortable using tablets, smartphones, mobile applications, and electronic documentation systems. Excellent communication and interpersonal skills.
