Support Services Specialist Washington County Commission on Aging, Inc. 535 East Franklin Street Hagerstown, MD 21740 Full-time Non-Exempt Position; 37.5 hours per week Monday - Friday 8:30 am - 4:30 pm The Washington County Commission on Aging / Area Agency on Aging seeks a full-time employee to fill the Support Services role to serve older adult clients in Washington County.
Responsibilities include:
Assist individuals and families in determining eligibility for home and community-based services.
Guide individuals in navigating and accessing appropriate resources and supports.
Support clients in adopting healthy behaviors through goal setting, motivation, and advocacy.
Identify and connect clients with available community resources.
Provide encouragement and ongoing support to help clients achieve their goals.
Maintain positive, collaborative working relationships with staff, partner organizations, and community agencies.
Demonstrate initiative and the ability to work independently with minimal supervision.
Adapt to a fast-paced and changing work environment, managing multiple tasks effectively.
Uphold professional standards of communication, confidentiality, and ethical conduct.
Requirements:
High School Diploma/GED General Studies. Associates degree in Human Services field preferred.
Community Health Worker preferred.
Three or more years of relevant experience with case management and mental health preferred.
Quick to adapt to rapidly changing environment and ability to learn new skills easily.
Basic computer experience is required.
Must have a valid driver's license with insurance.
Requires the ability to travel throughout Washington County using personal vehicle.
Must meet the requirements of a background check.
Physical and Drug Screen.

This full-time position offers coverage to include Health Insurance, Life Insurance, Dental, and Vision. Pay:
$17.00 - $17.50 per hour Expected hours: 37.5 per week
