Family Medical Center of Michigan is seeking an experienced Full Time Community Health Worker for a one year grant based position in our Adrian, Michigan location. Family Medical Center of Michigan, Inc is a health center offering dental, optical, medical and behavioral health services.
NATURE OF WORK
The Community Health Worker works with patients, care teams and community resources to coordinate and support chronic disease management and wellness initiatives through education, prevention initiatives and referrals.
GENERAL DESCRIPTION OF DUTIES
1. Greet pre-identified patients upon arrival and provide hands-on assistance with kiosk-based or mobile online check-in to improve comfort with digital registration, reduce barriers to access, and support efficient clinic workflow. 2. Assist patients with activating and using the patient portal, including online appointment scheduling, review of laboratory results, medication refill requests, provider messaging, and access to individualized care plans. 3. Educate and assist patients in completing electronic Social Determinants of Health (SDOH) screening tools during visits and help identify needs related to food insecurity, transportation, housing instability, financial hardship, behavioral health, medication adherence, or other barriers to care. 4. Provide real-time resource navigation, referrals, follow-up, and warm handoffs to appropriate healthcare and community-based resources for identified SDOH and care needs; track referrals and support patient follow-through. 5. Participate in daily care team huddles and collaborate with providers, nursing staff, care coordinators, and front office teams to identify patients who may benefit from enhanced support before, during, or after their visit. 6. Meet with patients following provider visits to reinforce care plan instructions, support chronic disease self-management, identify barriers to treatment adherence, and coordinate next steps in care management. 7. Assist patients with scheduling follow-up appointments with primary care providers or specialists and support appointment adherence by identifying and addressing barriers that may prevent completion of recommended care. 8. Support real-time care gap closure by helping identify patients due for preventive screenings, immunizations, annual wellness visits, laboratory monitoring, or chronic disease follow-up and coordinating with the care team to complete services whenever possible during the same visit. 9. Support grant implementation and reporting by tracking, documenting, and providing timely updates on key grant measures, performance indicators, activities, and outcomes as requested by program leadership. 10. Adheres to and supports the policies, procedures and mission of the Family Medical Center as well as adhere to local, state and federal guidelines. 11. Keeps and maintain accurate records, complete reports and other paperwork as necessary in a timely and efficient manner. 12. Performs other duties as directed by the CHW/Population Health Supervisor or designee. Family Medical Center of Michigan offers a $300. sign on bonus, benefits at 30 days if elected, accrued vacation and sick time, no weekends or major holidays or late evenings.
QUALIFICATIONS
: 1. High School Diploma or equivalent. 2. Experience in excellent customer service delivery. 3. Previous experience in a health care setting preferred. 4. Previous experience with electronic health record systems. 5. Valid drivers' license and current insurance. Must be able to drive to other FMC locations in Monroe County on infrequent occasion. 6. Excellent skills in communication (written and verbal) with the ability to impart information and relate effectively to patients, providers and other team members. 7. The ability to organize activities, keep accurate records, and complete reports as needed. 8. The ability to carry out the assigned duties of this position independently or with only minimal supervision. 9. Community Health Worker certification is not required upon hire. The employee will be required to work toward CHW certification during the first year of employment, with certification-related costs covered by the employer.
