JOB SUMMARY
Under the supervision of a higher classified employee, the Community Health Worker (CHW) will perform a variety of outreach and customer service activities in both an office and non-office setting. The CHW role exists to build strong, mutually-beneficial relationships withmunity members. These connections can increase access to available health services and can support changes to programs and services w needed to better supportmunity health for all. Additionally, the CHW may provide support to all divisions within the Health Department, such as acting as a Health Department representative in a variety ofmunity settings. This position will support the Children's Special Health Care Program.

EXAMPLES OF DUTIES
Essential Duties:
Provides client-centered support for Washtenaw County Health Department programs and services in amunity, home or clinical setting.
Some examples may include:
munity lead testing, direct observed therapy, program enrollment, translation or other support as needed. Supports case management goals by working to include client voice and preferences in planning. Participates in the planning and implementation of client care plans. Mayplete home ormunity visits, alone or with other staff, to provide services or engage withmunity members. May teach skills needed to increase independence in the areas of work skills, safety skills, social skills, finding and maintaining housing, and receiving and maintaining benefits. Works to secure needed services for the clients from any health, mental health, or social service provider. This may include advocating for systems changes that better support clients' health or needs. Providesmunity education and outreach for health department services. Develops relationships withmunity partners to support health and the values outlined in the department's strategic plan. Provides clerical support as needed. Provides services in a culturallypetent manner.
Examples may include:
providing written and spoken materials in clients preferred language and utilization of interpretation services. Participates in individual, program and department processes with the goal of continuous quality improvement. Provides timely and accurate documentation for services provided. Updates and maintains a list ofmunity services and agencies for referral purposes. Performs related duties as assigned. EMPLOYMENT QUALIFICATIONS
Knowledge, Skills and Abilities:
Ability to understand and an array of department and county policies, procedures, rules and regulations. Ability toanize, prioritize and coordinate work with minimal supervision. Ability to document interactions using writtenmunication with clear English and proper grammar, spelling and punctuation. Ability to use a wide variety ofputer software including, but not limited to, Microsoft Office products, Outlook and social media such as and . May receive additional training on state and department required systems. Ability to work cooperatively in a team and/ormunity setting while establishing and maintaining effective working relationships. Ability to provide quality client centered services under changing and challenging circumstances in an accepting and non-judgmental way. Ability to maintain client confidentiality.
DESIRED QUALIFICATIONS
Proficiency in a second language. Community Health Worker Certification recognized by the State of Michigan. Strongmunity connections and/or similar life experiences to themunity or population served. Experience working with people. Experience working with children with special needs is desirable. Parent of a special needs child is desirable.
LICENSES AND CERTIFICATIONS
Possession of a valid Michigan vehicle operator's license. Availability of automobile, van, or truck.
PHYSICAL DEMANDS
Duties require sufficient mobility to work in a both a normal office setting and in themunity. Use of standard office equipment including as well as specialized devices tomunicate with hearing and speech impaired clients. Requires ability to read ed material and must be hearing and speech sufficient tomunicate in person or over the phone. Requires ability to drive to multiple work locations. These requirements may be amodated for otherwise qualified individuals requiring and requesting such amodations.
EDUCATION/EXPERIENCE
Possession of a high school diploma or equivalent. This is an entry-level classification; no specific prior experience is required.
