Part-time Community Health Worker (CHW)

Job Description

The Tuscola County Health Department is seeking a Community Health Worker (CHW) to work on a contractual basis. This position is under direct supervision of the Health Officer. This position plays a pivotal role in local public health programs by meeting clients where they are to provide one to one support, targeted outreach, health education and navigation, referrals and peer support. The CHW position builds trust with clients and other health and social services providers to connect clients to services addressing their social determinants of health (basic needs). As a priority, activities will promote, maintain, and improve the health of clients and their families. More information can be found at www.tchd.us Job duties may include but not limited to: Promote and maintain awareness of health department programs, services and activities. Address social determinants of health by assisting clients in eliminating barriers to healthcare. Survey/and or screen clients to identify needs for community resources, food, housing, transportation, economic stability, social/community environment, and healthcare. Assist client in obtaining access to health and social services based on their eligibility. Provide care coordination and system navigation to assure client access to care. Work collaboratively with internal staff personnel/programs to provide integrated care. Provide health education classes and/or support groups. Advocate for individual and community health needs. Act as a liaison between communities, individuals, and other service providers. Provide a culturally competent bridge among individuals, communities, social service systems, and health systems by providing appropriate health education and information. Document activities, outreach, and needs required by program procedures. Timely complete all forms and required reports including all documentation of outreach attempts, service(s) and /or education provided, referrals, or other pertinent information. Attend relevant classes and trainings. Attend community meetings, health fairs, and other events to understand community needs and provide information to community members. Work well independently, as a team member, and collaboratively with health and service providers. Support the mission and vision of the Tuscola County Health Department as well as develop and continue partnerships with organization that interface with the program.
Requirements:
High school diploma or equivalent; or a college degree in either social work, health and human services, or health education Community Health Worker certification or willingness to become certified One year (+) of experience of working with community agencies, providing services encompassing housing, food, healthcare, etc.. Knowledge of community resources MUST possess a valid MI driver's license without limitations. A drug test will be required.
Wage:
$20.00 per hour (no benefits) Typical agency hours are Monday-Friday 8:00 a.m.-4:00 p.m.
